National Council of Licensure Examination (NCLEX)

National Council of Licensure Examination (NCLEX) and State Board Application Information

Indiana State Board Application: Due to deadlines set by the Indiana State Board of Nursing, State Board applications will be collected from each student by your instructor.  Students will be notified in class or via email of the deadline.

The applications will then be sent as a group to the State Board.

  • ID photos are required to be attached to your State Board applications. ID photos will be collected in advance of the State Board application deadline.  ID photos require a school seal and signature of the applicant and nursing department chair.  The signed and sealed ID photos will be distributed to students the day the applications are due. 
  • A check in the amount of $50.00 is also required as part of the application.
  • Please note: Page 3 of the application will be sent by the department at the completion of Virtual ATI. Do not include page 3 with your application.
  • Please make every effort to have your application ready by the deadline.

Virtual Assessment Technologies Institute (ATI) information:

  • When you are ready to begin Virtual ATI, the Department of Nursing will send your name to ATI.
  • You will be contacted by ATI through your gmail email.  If you do not have a gmail account, you can set one up by going to google.com and clicking on gmail.  Then update your profile at atitesting.com with your new gmail email address.
  • ATI will send information for you to complete via your gmail address
  • ATI will contact you within two days of your returning the completed information.
  • The Nursing department will receive a weekly progress report every Thursday.
  • Upon your successful completion of Virtual ATI, the Nursing department will send your Certificate of Completion to the Indiana State Board of Nursing.
  • 12-week access: students will have access to Virtual-ATI for 12 weeks, beginning once the student has completed their initial survey and is within 30 days of graduation. This will encourage students to review and sit for NCLEX sooner leading to higher pass rates and retention of knowledge. The student can purchase an additional 12 weeks, if necessary.
