IU Course Change Form

To request a change in an IU course, please complete the form electronically.

Once your proposal is completed:

  1. Please send the form to your chair, via email, for approval.
  2. The chair needs to forward it to the dean office for approval.
  3. Finally, the approved form should be forwarded to Douglas Townsend at: townsend@ipfw.edu .
