NEW Regional Forms for Junior Division Grades 6-8

1. Did you answer the four (4) questions from the Pre-Experimentation Review Questionnaire? If not, do that first by clicking Online Eligibility Forms. This will determine if your project requires "pre-experimentation review" or is "exempt" from pre-review. NEW FOR 2010: Hazardous chemicals, devices and activities are prohibited by the Science Education Foundation of Indiana, Inc. (SEFI) for grade 6-8 projects.

2. If your project requires pre-review, submit your approval form, research plan, and form that applies to your project before Jan. 27, 2010 for SRC approval. Do NOT start your project until you have received SRC approval.

3. If you project is exempt from pre-review, your approval form, research plan, and exemption form are not submitted to IPFW until you are chosen to advance to the regional fair. See #4.

4. ALL STUDENTS who advance to the regional fair must submit an approval form, research plan, the pre-review form that may apply, and an abstract by March 5, 2010. Abstracts are written after your experiment is done.

5. Students may print forms and hand-write information or complete them online, save and print. NOTE: When saving, change the file name to prevent overwriting file.

6. Print and sign regional forms before submitting.

Approval Form Required for ALL grade 6-8 regional fair projects
The Approval Form is required for all pre-review projects and for exempt students that advance to the regional fair. The student, teacher, and parent sign this form. We recommend that exempt students complete the approval form with signatures before they begin their project. In the event they advance, their form will have proper signatures and dating.

Research Plan A research plan is required for ALL grade 6-8 regional fair projects. You may use this form or submit your own written research plan.

For projects using humans as subjects (includes consent form)
Wait to get your consent forms signed until after the SRC approves your project.

Pre-review form:Vertebrate animal grades 6-8
Used for projects involving vertebrate animal research

Pre-review form:Risk assessment grades 6-8
Used for projects with potentially hazardous biological agents
