Transcripts

The transcript request forms on this page must be completed, signed, and mailed or faxed 260-481-6110 to the Office of the Registrar. Your signature is required to process any transcript request.

Indiana University Official Transcripts

The Office of the Registrar (Kettler Hall Room 107) can provide an official Indiana University transcript for IU students who have been enrolled at IPFW or any other IU campus. There is a $4.00 charge for this request. If you attended any IU campus prior to 1966, please contact the Office of the Registrar at Indiana University—Bloomington campus for transcript information.

Payment by Check or Money Order

To request an Indiana University official transcript:

  1. Complete and print the Request for Official IU transcript Form.
  2. Sign the request form.
  3. Make your $4 check or money order payable to IPFW.
  4. Write your Student ID Number on your check or money order.
  5. Mail your check or money order and the request form the Registrar's office.

Payment by Credit Card

To request an Indiana University official transcript:

  1. Complete and print the Request for Official IU Transcript Form.
  2. Sign and mail the form.

Purdue University Official Transcripts

The Office of the Registrar (Kettler Hall Room 107) can provide an official Purdue University transcript for Purdue students who have been enrolled since 1990 at IPFW or any other Purdue campus. There is a $4 charge for this request. If you attended Purdue University prior to 1990, contact the Office of the Registrar at Purdue University—West Lafayette campus.

Payment by Check or Money Order

To request an official Purdue transcript:

  1. Complete and print the Request for Official Purdue Transcript Form.
  2. Sign the request form.
  3. Make your $4 check or money order payable to IPFW.
  4. Write your Student ID Number on your check or money order.
  5. Mail your check/money order and the request form.

Payment by Credit Card

To request an official Purdue transcript:

  1. Complete and print the Request for Official Purdue Transcript Form
  2. Sign and mail the form

IU or Purdue Unofficial Transcripts

To request an unofficial transcript for either Indiana or Purdue:

  1. Complete and print the Request for Unofficial transcript Form.
  2. Sign the request form.
  3. Mail the request form.

If you attended Purdue University prior to 1990, contact the Office of the Registrar at Purdue University—West Lafayette campus. Your transcript will be mailed to you at the address we have on record. No transcript will be faxed.

IPFW Unofficial Transcripts via myIPFW

You can view your IPFW unofficial transcript online at myIPFW. For security reasons we do not print your student id number on this transcript.
