Enrollment Certification

If a letter of enrollment certification is needed for your health insurance, car insurance, loan deferment, scholarship awards, or employer tuition reimbursement, follow these simple instructions:

  1. Print and fill out the Request for Enrollment Certification Form.
  2. Sign and mail or fax the request form to the Office of the Registrar.

Please include any other documentation that was supplied by the requestor.
