How do I change my address and/or phone number?

Students may change their address and phone number in OASIS under Personal Information. Students may also change their address and phone number by completing a Personal Information Change form in person at the Registrar's office. All students are required to have an active permanent address on file.

How do I change my name?

In-Person Requests:
Students whose names have been legally changed may come to the IPFW Office of the Registrar, Kettler Hall, Room 107, and file a Change of Name form. Legal documentation, such as an original social security card, marriage certificate, or official court document, must be provided to support the name change. This name change will be reflected on the student's academic record.

Mail, or Fax requests:
Complete the online Change of Name form and send your name change request by mail, or fax.

The following information should be included in your request:

    • Your 9-digit IPFW Student ID Number (or the last four digits of your SSN)
    • Exact name that now appears on your university record
    • "New"name as you want it to appear on your university record
    • Phone number or email address at which you may be reached for verification of submitted information
    • Your written signature
    • Reason for change (i.e. marital status change)
    • Copy of legal documentation, such as a copy of a social security card (only if submitting by mail; do not fax the SSN card), copy of marriage certificate, or official court documment that supports the name change.

What is my student ID number?

You will be assigned a 9-digit number typically beginning with either 900 or 999 as your student identification number. It is used to identify records within IPFW and has no significance outside IPFW. It will not be provided to external agencies or individuals except in accordance with university policy on release of student information.

I thought my student ID number was my social security number. What is it?

IPFW does not use your social security number as your student identification number except for those purposes required by law or governmental agencies (i.e. Internal Revenue Service, Free Application for Federal Student Aid). What must I do to make a correction to my name or social security number?

Students must have legal documentation (i.e., driver’s license, marriage certificate, or divorce decree) to make a correction to the spelling of their name. To make a correction of their social security number, students must present their social security card and also their driver’s license. These changes can be done in person at the Registrar's office by completing a Personal Information Change form.

What is directory information?

Directory information is student information which may be disclosed or publicly shared. Directory at IPFW includes:

  • Name
  • Address
  • Phone Number
  • Email Address
  • Class Standing
  • School/Division
  • Major
  • Dates Attended
  • Current Enrollment Status
  • Degrees and Awards
  • Recognized Activities/Sports
  • Information related to Athletic Team Participation

How do I make my directory information confidential?

Students who are currently registered may declare their directory information confidential by completing a Request To Restrict Directory Information form at the Registrar's office.

What should I do if I am not receiving my bills and grades?

Bills are sent to your IPFW e-mail address. You will not receive a paper bill at your mailing address. If you prefer to receive your bill at another e-mail address, go to myIPFW and set up e-mail forwarding to your preferred e-mail address.

Grades are sent to IPFW students from the main campuses in Bloomington or West Lafayette. If you are not receiveing your grades, verify that your mailing address is correct in OASIS or in person at the Registrar's office. If your mailing address is correct in the IPFW system, request that the Registrar's office verify that your mailing address is correct in the Indiana University system or the Purdue University system.

My records have been subpoenaed, but I don't want anyone to have them. How can I prevent the university from turning over my records to the court?

The university must comply with any and all subpoenas for records or it may be found to be in contempt of court. A subpoena for records can be quashed, but the quash is a motion of the court. Consult with your attorney. If you notify us that proceedings for a quash are in progress, you must subsequently forward to us the court motion to quash. If the university does not receive official court documents of a quash, it is bound to comply with the demands of the original subpoena.

Why won't you give my parents information about me?

The Family Educational Rights and Privacy Act (FERPA) is a Federal law that protects the privacy of student education records.
