Clinical Requirements
Health and clinical records must be turned in to the College of Health and Human Services Student Success Center (SSC) in NF 120. For any questions regarding clinical requirements please contact Lauri Haskins, the secretary for the SSC, at haskinsl@ipfw.edu or 260-481-4187.
A student who has not met clinical agency requirements within the required time frame of August 15 (fall semester), December 15 (spring semester, May 15 (summer I and II) will be dropped from clinical course(s) and any co- requisite course(s). Being removed from the course(s) may result in forfeit of financial aid.
Neither class or clinical space will be held for any student dropped by the deadline of August 15 (fall semester), December 15 (spring semester, or May 15 (summer I and II), therefore, if the class is full you will not be able to register.
THIS POLICY WILL BE FOLLOWED BY THE DEPARTMENT OF NURSING

Requirements for Veteran's Administration Hospital and Clinics:
| Complete and sign VA Criminal Background Check |
| Read and sign VA National Rules of Behavior Document |
| Complete Modules in EES learning |
| Complete and Sign VISN 11 Authorization for AIS Access |
| Complete Request for Personal Identity Verification Card |
Make a copy of all VA documents, turn copies in to the Graduate Secretary, IPFW Department of Nursing.
