Application Instructions
Online Option
If you are applying for licensure that falls into one or more of the following categories, you may apply online at https://pcats.doe.state.in.us////apps/psb/pcats/ . For any other license request, use Mail-In Option.
- Original Rules 2002 license
- Addition to Rules 2002 license
- Renewal and/or Addition of Rules 46-47 license
- Converting to a Professional Rules 46-47 license (Teaching License Only)
- Converting to a Professional Rules 46-47 license with Addition (Teaching License Only)
(Free Limited criminal history report included with On-Line application.)
Note: Applicable transcripts, CPR card if applying for an original license, CRU certificates, and employment verification letters must be submitted to the IPFW licensing advisor prior to submitting your online application request.
Mail-In Option
You may apply for a license through IPFW by mailing or by dropping off the application materials to the licensing advisor. All completed license applications MUST be submitted to:

(If you are renewing a Rules 2002 license– see section - RENEWAL OF AN INITIAL RULES 2002 (2 YEAR) LICENSE for separate instructions).
After the Licensing Advisor recommends you for your license, your forms will be sent directly to the Indiana Department of Education Division of Educator Licensing/Development. You should receive your license in approximately four weeks.
INITIAL PRACTITIONER (Original) LICENSE:
Submit the following to the Licensing Advisor:
- Completed license application (State Form #9331).
- Unofficial IU or Purdue transcript showing DEGREE STATEMENT. You must go to the Registrar=s Office (Kettler 107) to order your transcript. If you are obtaining a license by Acertification only@, submit a final transcript verifying completion of ALL COURSEWORK.
- $35.00 money order or cashier’s check payable to the State of Indiana. NO personal checks.
- Statewide Criminal History Report - the original letter or web report from the Indiana State Police (Indianapolis, Indiana).
- Photocopy of test scores (PPST, Specialty Tests, SLLA) if applicable.
- Photocopy of a valid CPR certificate card (both sides of card)
STAPLE items (a - f) with money order/cashier’s check on top
Note: If applying for a Rules 2002 Building Level Administrator license, you must also provide a letter from your school superintendent or human resources department verifying at least 2 years of classroom teaching experience.
ADDITION TO LICENSE or RENEWAL OF RULES 46/47 AND BULLETIN 400 LICENSE:
Note: License Renewals should not be made sooner than 60 days before the expiration date on the license.
Submit the following to the Licensing Advisor:
- Completed license application (State Form #9331).
- Transcripts showing 6 credit hours for renewal OR all coursework needed to receive an addition to your license. We require official transcripts if coursework was taken outside the IU or PU system. You may submit unofficial IU or PU transcripts which can be picked up at the Registrar’s Office (Kettler 107).
- $35.00 money order or cashier’s check payable to the State of Indiana. NO personal checks.
- Current license - original if valid, or a copy if license has expired.
- Statewide Criminal History Report - the original letter or web report from the Indiana State Police (Indianapolis, Indiana).
STAPLE items (a - e) with money order/cashier’s check on top.
PROFESSIONAL (10 YEAR) LICENSE (Rules 46/47 or Bulletin 400 licenses only):
- You must have completed your master’s degree and have verified five years of appropriate experience to qualify for the Professional license, which includes a one-time, 10-year renewal. Depending on the areas on the license, you may need additional graduate courses as well. Check with the Licensing Advisor to verify the number of credit hours needed to professionalize the license.
Note: You must complete 6 credits during your final 5 years of the professional license to renew.
- Submit the following to Jim Beard, IPFW Licensing Advisor:
- Completed license application (State Form #9331).
- Transcripts showing all coursework taken. You must provide official transcripts for coursework taken outside the IU or PU system. You may provide unofficial IU or PU transcripts. They may be picked up at the Registrar’s Office (Kettler 107).
- $35.00 money order or cashier’s check payable to the State of Indiana. NO personal checks.
- Current license - original if valid, or a copy if license has expired.
- State Criminal History Report - the original letter or web report from the Indiana State Police (Indianapolis, Indiana).
- Experience Verification Letter(s) - must be completed by your school superintendent on SCHOOL LETTERHEAD.

RENEWAL OF AN INITIAL RULES 2002 (2 YEAR) LICENSE
Note: License Renewals should not be made sooner than 60 days before the expiration date on the license.
Submit the following directly to:

- Completed license application (State Form #9331).
- $35.00 money order or cashier=s check payable to the State of Indiana. NO personal checks.
- Current license - original if valid, or a copy if license has expired.
- Statewide Criminal History Report - the original letter or web report from the Indiana State Police (Indianapolis, Indiana).
- Letter from you stating you have not completed your Indiana Mentoring and Assessment Program (IMAP) internship.
STAPLE items (a - e) with money order/cashier’s check on top
Note: You can only renew this license twice without renewal credits. After the second renewal (6 years) you must show 6 credits taken within the previous 5 years to renew the license. You will need to provide transcript(s) showing the 6 credits of coursework in your content areas or in education along with the items listed above (a-e) to Jim Beard, Licensing Advisor (see address above). Once you renew your license a third time, the 2-year renewal cycle starts over.
