Criminal History Report
Overview
On July 1, 1997, the Division of Professional Standards instituted the Limited Criminal History check for all actions to Indiana teaching, administrative and school services licenses.
Your original Criminal History Report must accompany your license application.
The Criminal History Report is valid for one year from the date of issuance, regardless of the number of licenses requested within the one year.
You can obtain your Limited Criminal History Report by mail following these steps:
- Fill out Request for Adult Criminal History Information form available in the School of Education Advising/Licensing Office Neff Hall, Room 243 or on the internet at: https://secure.in.gov/apps/isp/lch/
- Complete all information under SUBJECT of Request. List your name, mailing address and daytime phone number. If student teaching, check box 1 under Reason for Request. If applying for license, check 2 under Reason for Request. Sign and date the form on the back.
- Obtain a $7.00 Money Order or Cashier's Check (payable to "State of Indiana").
- Mail these items to:

Indianapolis, IN 46206-6188
Your Criminal History Report will be mailed to YOU.
Online Limited Criminal History Reports can be obtained at www.in.gov/isp/lch

Note: A fee will be charged to your credit card.
Complete all information under requesting individual and subject of request.
Under reason for request:
- if student teaching, check #11
- if applying/renewing license, check #1
