Student Health Insurance
Purdue Regional Campus Eligibility:
All registered Domestic and International* students are eligible to enroll in this insurance Plan. All insured students may purchase Major Medical coverage on an optional basis. Students must actively attend classes for at least the first 31 days after the date for which coverage is purchased. Home study, correspondence, Internet, and television (TV) courses do not fulfill the eligibility requirements that the student actively attend classes. Please see the 2008-2009 insurance plan brochure for further information.
*IPFW International students are NOT eligible for this insurance. Our International students are required to have a specific insurance unique to the IPFW campus. You MUST contact the office of International Student Services at 1-260-481-6923 or via email at iss@ipfw.edu. Their office is located in Kettler Hall, room 104.

You may enroll and pay for your student insurance online. Online enrollment takes only a few minutes and allows you to pay via credit card or echeck.

Enrollment via Postal Mail:
If you would like to complete your enrollment form via postal mail please visit the following link and mail to the address listed on the enrollment forms. Be sure to save a copy of your enrollment forms for your own records.
2008-2009 Enrollment Forms
Continuation Privilege:
All Insured Persons who have been continuously insured under the school's regular student policy for at least 6 consecutive months and who no longer meet the Eligibility requirements under the Policy are eligible to continue their coverage for a period of not more than six months under the school's policy in effect. Please see the 2008-2009 insurance plan brochure for further information.
