The IPFW Lafayette Street Family Health Clinic is a nurse-practitioner run clinic with a patient centered model of care.
We welcome women and men from all cultural and ethnic backgrounds and have a translator available at no cost for our Hispanic clients. Hablamos Espanol!
The clinic fees are based on income and family size for all aspects of family planning.
The clinic is funded through the Indiana Family Health Council by the U.S. Department of Health and Human Services, Public Health Service, Title X Family Planning Project.
Location
The clinic is located in the Lafayette Medical Center, 2700 S. Lafayette Street, Fort Wayne, IN 46806, on the second floor in suite 200. The building is handicap accessible and parking is available.

The clinic is open:
Monday | 8:00 AM to 7:00 PM |
Tuesday | 8:00 AM to 4:30 PM |
Wednesday | 8:00 AM to 5:30 PM |
Thursday | 8:00 AM to 4:30 PM |
Friday | 8:00 AM to 12:30 PM |
Closed daily for lunch from 12:00 to 1:00 PM

- Stop by the reception desk in the Clinic lobby
Patients are seen by appointment only. Appointments are scheduled on the basis of availability. We encourage you to call ahead at 260-481-0400. It is important to arrive on time. If this is your first appointment you will be asked to complete a medical history form.
Staff
The clinic is staffed with nationally certified nurse practitioners and certified medical assistants. Our collaborating physician is Dr. John E. Drake.
