M.S. Clinical Requirements

Health and clinical records (TB certification, physical, etc.) must be turned in to the Nursing Office LA 345 by the deadlines listed below. Documentation must be good through the entire semester.  if you are not up to date with the requirements you may not begin clinicals. For any questions regarding clinical requirements please contact the nursing office, 260-481-6816.



Fall semester - August 15

Spring semester - December 15

Summer semester - May 1

Required Preceptor Documents

You will submit your preceptor forms to a dedicated Blackboard site.  The site contains the preceptor forms along with information about preceptors.  The preceptor list is updated periodically.

Deadlines are for submitting preceptor forms are:

  • May 1 (fall semester)
  • October 1 (spring semester)
  • February 15 (summer I and II)

FNP and AGNP students should seek practicum experiences in primary care settings that match the focus of the course - IPFW Graduate NP Practica Guidelines
