Health and clinical records (TB certification, physical, etc.) must be turned in to the Nursing Office LA 345 by the deadlines listed below. Documentation must be good through the entire semester. if you are not up to date with the requirements you may not begin clinicals. Fall semester - August 15

You will submit your preceptor forms to a dedicated Blackboard site. The site contains the preceptor forms along with information about preceptors. The preceptor list is updated periodically.

Clinical requirements must be turned in to the Nursing Office in LA 345 by the policy deadlines. Requirements may also be scanned and emailed to Jeni at email@example.com or Sarah at firstname.lastname@example.org with the exception of the drug screen results. Drug screen results must be received in a sealed envelope -- either dropped off by student, or mailed directly from facility to the nursing department.
