Admission Procedures

  1. Apply for admission to Indiana University through the Admissions office in Kettler Hall, Room 111 at 2101 E. Coliseum Blvd., Fort Wayne, IN 46805-1499.
  2. Take the English, reading and math placement tests, unless you are a transfer student and have received credit for Math 113, or higher on your IPFW Credit Transfer Report. Call the Collegiate Connection office at 260-481-6600 for testing dates and procedures.
  3. A separate application for the dental laboratory technology program must be postmarked by Monday, June 12, 2010 to apply for the Fall 2010 semester.
  4. The following are prerequisite course requirements and need to be completed with a grade of C or better by June 26, 2010 for admission into the fall 2010 program. Final transcripts must be postmarked by June 12, 2010, unless an IPFW summer I 2010 course is being taken, to be considered for the fall 2010 class.
    COM 114, Fundamentals of Speech (Gen Ed Area I)
    ENG W131, Elementary Composition (Gen Ed Area I)
    Other Prerequisite Course Requirements-
    1) Must pass all prerequisite courses with a “C” or better
    2) If you are getting anything below a C in any course, withdraw from that course before the Deadline Withdraw Date (see the Schedule of Classes for the exact date each semester). Then, complete the course again next semester
    3) Must have a minimum prerequisite GPA of 2.0/4.0
    4) Must also have a minimum cumulative GPA of 2.0/4.0
  5. A personal interview with the dental laboratory technology admissions committee may also be required to determine final class selection. After completion of steps 1, 2, and 3, an interview appointment (if necessary) will be made with the admissions committee.

    In addition to the Dental Laboratory Technology classes, students must also complete the following (these courses may be taken prior to entering the program):
    PREFERRED ADMISSION:
    BUS W100 3 Credit Hours
    One of the following: STAT 125, MA 153 or MA 149
