Health Fees/Insurance
Our clinic is a fee-for-service health facility. Students at IPFW are not currently assessed a student health fee each semester. We request payment at each visit by cash, check, or credit/debit card.
Please be advised, as with all insurance we will file a claim for you. However, there are no guarantees for payment of service. Ultimately, it is the patient's responsibility for any or all services that might not be covered by their insurance.

Please bring a copy of your insurance card with you to each visit.
Upon checking out, an insurance-ready itemized statement will be provided to you so that you may submit it to your insurance company.
