How do I Enroll?

Enrollees will be accepted without regard to race, religion, national origin, sex, or handicap. Children must be at least 2 years of age and no older than 12 to attend the center. Children are enrolled for the semester and must re-enroll each semester or summer session. Children must be enrolled no later than the Thursday before the start of the semester. If enrolled after Thursday, the child will be required to wait one week to begin attendance.

To enroll your child in the Child Care Center, you must:

  • Complete the appropriate enrollment form and pay the required fees as listed on the enrollment form. This will hold your child's space while you complete the remaining paperwork.

Before your child may attend you must:

  • Complete forms 1-5
  • Submit a copy of your child's birth certificate
  • Submit a copy of your class schedule
  • Return completed Child Immunization Record with physician's signature

Within thirty days of enrollment you must:

  • Return the completed Health Record with physician's signature

To enroll your child you may either come to the Child Care Center at 4133 Hobson Road, Fort Wayne, IN or complete the forms provided below and mail with the required fees to:
