Photography⁄ Observation Permission

Because the center is part of a university, it is used from time to time by students needing to observe children at play. Your child will never be tested, only observed. Photographs and videotapes used for promotional and/or educational purposes are taken periodically by various campus departments. If you wish to withdraw your authorization for your child to be photographed, please return this form, mark through the release information, write a statement across the top of the form, and sign your individual statement.
