Immunizations

For the protection of all children attending the center, immunizations must be up-to date prior to enrollment. The Immunization Form [PDF] must be completed (including dates of immunizations) by your child's physician or the local board of health nurse. Updated immunization records are required annually to be turned in to the Center, regardless of whether new immunizations have been added or not

Note: Your child may not begin attending unless the completed child immunization record is returned to us.
