Health Record
Within 30 days after enrollment each child shall have a written verification of a physical examination by a physician, or a nurse practitioner. The Health Record must be no older than 12 months prior to the enrollment date.
The examination will include the following:
- Child's medical history
- Physical examination
- A written statement claiming that there is no health condition that would be hazardous either to the child or to other children in the child care center as a result of participation in the program of activities
Children under two years of age must have an annual health examination. Children two and over are not required to have an annual physical unless they are new enrollees to the Center.
Health records may be transferred or copied from another child care center or school if the physical exam is dated within one year.
