Emergency Information
It is very important that the Emergency Information Card [PDF] be filled out completely and carefully. Please complete both sides. Individuals attempting to pick up your child will be required to produce an acceptable form of identification. There will be no exceptions to this rule. If an emergency arises and someone other than those listed on the card needs to pick up your child, we must be notified by phone. If not, we will not be able to release the child. Please keep us informed of any important changes, such as telephone numbers and addresses, which need to be made this afternoon.
