Child Care Enrollment Form
Upon receipt of the Child Care Enrollment Form [PDF] and the registration fee, you will be given a handbook and other forms which need to be returned to complete the enrollment process.
With the enrollment form, include a copy of your IPFW (or other university) class schedule, a copy of your child's immunization records, and the appropriate fee.
